Appointments

 

Each patient, whether referred by an insurance health plan, family member or practitioner, is first scheduled through the office by one of our experienced and competent staff. Our staff will walk the patient or patient representative through our prescreening questionnaire in order to ensure that the patients needs and concerns are properly documented and addressed. Appointments are based on our physicians’ schedules and the patient’s needs. Our office staff will contact the patient prior to the appointment date for confirmation of clinic visit.

New Patients

Before scheduling new patients, the following information is crucial in order for our staff to establish the patient in our system and properly address concerns.

  • Patient’s Full Legal Name
  • Patient’s Insurance Information
  • Patient’s Current Phone Number and Address
  • Medical Condition i.e. any major health history, recent admissions and/or reason patient requires appointment
  • Patient’s Current Medication(s) (state if refills will be needed)
  • Information on Name, Address & Phone Number of Patient’s Past Medical Facility (provider, clinic and/or hospital’s contacts)

Insurance
